I also almost always use medium, the Lahmian kind, as I'm still on my first pot and that was the easiest to obtain. When I run out, I'll probably get some more from an art store, or something. I usually use both water and medium, especially if the paint is very thick. But for very delicate job, like edge highlights and pupils, I omit the water and use only medium. And, on the other hand, for base coating, I use just water to thin the paint down.

EDIT: 666 messages and a picture of a goat! Metal!

So every time you take a bit of paint from the pot, and put it on your palette you mix in a little bit of Lahmian?

On the topic of Lahmian Medium - is it a suitable thing to mix with normal GW paints to make them appropriate for use in an airbrush, or is water better? I'm thinking along the lines of old Mechrite Red and whatever the equivalent of foundation paints are these days.

There is a dedicated thinner for airbrushing,
    http://www.amazon.co.uk/Liquitex-Professional-Airbrush-Effects-Medium/dp/B001US2NQA

    But yes Lahmian will work.
