Its from the Shadow of the Erdtree DLC for the game Elden Ring, produced by the company FormSoftware (who are known for producing difficult games such as the Dark Souls series). Elden Ring was already considered to be a difficult game, filled with difficult bosses. But the DLC's Consort of Radhan was more difficult than them all (so I've heard, I only own the base game). Eventually they nerfed the boss making him easier.
